License Applications for Nurses and Nursing Student Graduates

All license applications must be submitted online. The “sample” applications on this website are provided for reference purposes only.

If you currently hold a license with the Nursing Board, have applied previously, or have applied to another Ohio board or commission, click “I Have a License” when creating a new user account to see if you are already in the Ohio eLicense system.

If you have never applied for a license or certificate with the Nursing Board or any other Ohio board or commission, click “I Don’t Have a License” when creating a new user account.

New Graduates Seeking RN or LPN Licensure

You must complete an online Board application for licensure by examination and take the National Council Licensure Examination (NCLEX).  To request a copy of the NCLEX Bulletin contact Pearson VUE at 1-866-496-2539 or online at www.vue.com/nclex. The National Council of State Boards of Nursing (NCSBN) develops the NCLEX and Pearson VUE is the organization that administers the examination. LPNs who are licensed in Ohio to administer medications and want to become licensed to provide IV therapy must complete an approved Intravenous Therapy CE Course.

Nurses Applying for Advanced Practice Registered Nurse Licensure

You must have an active RN license in Ohio to apply for an APRN license. You can apply for an APRN license from your RN dashboard on www.eLicenseOhio.gov by clicking on “Options” on the RN license and “Apply for an Endorsement.”

Verification Form for Organizations Issuing APRN National Certification

Each year national organizations that administer APRN examinations and issue APRN national certifications must verify to the Board that the organization meets the statutory requirements for approval by the Board. (Section 4723.46(A) of the Ohio Revised Code.)

Out-of-State Nurse Applicants Applying for Licensure by Reciprocity

If you are licensed in another state that submits licensure data to the NCSBN NURSYS system, you must request verification online at www.nursys.com. If you do not know if the state in which you are licensed is part of the NURSYS system, you can find that information on that website as well. If the state is not in NURSYS, please click on the link below.

Application Status & Online Payments

You can check the status of your application on the Ohio Board of Nursing website at www.nursing.ohio.gov/Verification.htm. Just click on “verification” and follow the instructions.

All application fees must be paid online using a credit or debit card. If you do not have a personal credit or debit card, you can obtain prepaid cards at local stores. If the fee is not paid, the application will be incomplete and will not be processed.
